New Patient Registration
You can register with the practice providing you live within the practice boundary (S3, S4, S5 and parts of S9).
If you wish to register with the practice please complete a Registration Form. Please be aware it is one form per person. A copy of this can be completed on this website or downloaded and returned to the practice via the postbox or email firstname.lastname@example.org.
Medical treatment is available from the date of registration. Please contact reception for more information.
Your form will then be sent to NHS Sheffield CCG who will arrange for your medical notes to be transferred from your previous doctor; this would normally take 7– 8 weeks.
You do not need proof of address, identification or an NHS number in order to register with a GP surgery. If you do have these they may help us to find your records from your previous doctor but they are not required to register.
Our receptionists won’t ask you for proof of your immigration status.
If you have a family member staying with you who needs medical attention, they can be seen by a doctor as a temporary patient. Please download and complete the temporary registration form from this website.
All are welcome! Your nationality or immigration status do not affect your right to register here.
We are a Safe Surgery for everyone in our practice area.
- Everyone living in England has the right to free NHS care from a GP.
- Ask reception for an interpreter if you find it difficult to communicate in English.
- Our receptionists won’t ask you about your immigration status.
